Genesys HealthWorks continually garners national recognition for its accomplishments. Recently, in response to the focus on national healthcare reform, the Institute for Healthcare Improvement shared “Triple Aim” stories with President Obama’s administration and health policy leaders. HealthWorks was selected by IHI as one of five nationally significant programs to share due to HealthWorks’ success in improving the health of our community, creating unique partnerships, and achieving cost savings.

The HealthWorks Model in Action: Making a Difference in Patient Lives

Having Hope for the Future: One Patient’s Story:

Ms. G is a 51-year old female Genesee Health Plan (GHP) member who has suffered with chronic pain for the past 7-years. The Genesys HealthWorks Health Navigator contacted Ms. G and engaged her in a discussion about her health history and her struggles with chronic pain. During their conversation, the Health Navigator learned that Ms. G has been diagnosed with fibromyalgia. In addition, Ms. G struggles with hypertension and obesity, and feels sad and blue most days. Ms. G told the Health Navigator that she smokes one pack of cigarettes a day, is in need of physical therapy, and lacks the motivation to start exercising. After their discussion, Ms. G agreed to work on a goal of “managing her pain and improving her daily functioning.”

Over the next three months, the HealthWorks Health Navigator and Ms. G developed a relationship, and the Health Navigator supported Ms. G as she worked to achieve her goals and healthy lifestyle changes. After learning that Ms. G was interested in walking more for exercise, the Health Navigator mailed her a pedometer so she could track her progress. In addition, the Health Navigator worked with the staff at the Genesys Athletic Club to secure a free one-year membership for Ms. G, through the club’s scholarship program for GHP patients. The Health Navigator also connected Ms. G back to her primary care physician for assistance with her depression and helped her secure a referral for physical therapy.

During their three-month follow-up conversation, the HealthWorks Health Navigator learned Ms. G had been able to manage her pain better, started physical therapy, and had also adopted a healthy diet and exercise regime. At the end of their call, Ms. G told the Health Navigator that she feels better about herself and now has hope for the future.
