What is Cardiac Rehabilitation and Prevention?
  • The Cardiac Rehabilitation and Prevention program focuses on preventing and managing heart disease by promoting a healthy lifestyle.
  • The program includes an individualized exercise plan as well as lifestyle education and support on how to make life-long changes.
  • Patients who have one or more risk factors for heart disease, who have experienced some type of cardiac event (such as surgery or heart attack), or whose doctor thinks the program is necessary, are all eligible to participate.

The Program
  • Phase I
  • Only occurs if you require a hospital stay following a cardiac event. It is incorporated into your daily inpatient care.
  • Consists of low-level activities such as standing and walking, as well as basic lifestyle education from nursing staff.

Phase II
  • Begins with a comprehensive assessment about your risk factors.
  • Includes heart monitored exercise classes individually designed for each patient, conducted under the supervision of clinically-trained exercise physiologists and registered nurses.
  • Includes weekly education sessions on topics such as diet and nutrition, stress management, and cardiac risk factors.
  • Most insurance companies cover the cost of Phase II.

Phase III
  • Designed for patients who have completed Phase II, or for people who are at risk for heart disease.
  • Provides a framework for an on-going, life-long commitment to physical activity and living a healthy lifestyle.
  • Typically not covered by insurance, but includes an affordable self-pay program.

How do I get Started?
  • A physician's referral is required to participate in Phase II or Phase III of the program.
