What is CHOICES Care Navigation and Coordination?
When we face a serious health condition, the road to wellness may not always seem easy. Advanced or serious illness involves important decisions that are highly personal.

How do I get started?
CHOICES can help you with the issues and uncertainty you may face and help you find your way through the confusing maze of health care. The first step begins with your own decision to take charge of your illness and decide how you want to live your life. A specially trained nurse coordinator will guide you through this process and help you look at all of the options available. CHOICES is about creating positive days and your own goals for living.
