What is the CHOICES program?
When we face a serious, life threatening health condition, the road may not always seem easy. Advanced or serious illness involves important decisions that are highly personal. CHOICES is a Palliative Care program designed to give you and your family support in the following ways:
- To help you learn more about your illness
- To help you with complex decisions regarding the plan of care for your illness
- To help you achieve the best quality of life in the face of serious, life-threatening illness
- To use an interdisciplinary team approach in managing your pain and symptoms
CHOICES patients have shared:

How do I get started?
CHOICES can help you with the issues and uncertainty you may face and help you find your way through the confusion that comes with dealing with serious illness. A specially trained nurse will guide you through this process and help you look at all the options available. CHOICES offers a support system that will promote making the most of life while living with a serious illness.
