Medical Licensure and DEA

All trainees must obtain a State of Michigan Medical Training License and Controlled Substance License prior to beginning your program. Please note that the license requirements are different for an osteopathic trainee versus an allopathic trainee versus a podiatry trainee so it’s very important that you complete the appropriate license application below. Return the application and appropriate supporting documents to Medical Education by April 9, 2010, along with a check made payable to the State of Michigan in the amount of $170.00 (osteopathic and allopathic) and $135 for Podiatry. DO NOT return the application to the State of Michigan. You must have a valid license in place by June 30th, otherwise you will not be allowed to begin your first rotation and your contract end date will be extended as necessary. No exceptions! It takes 8 weeks to process applications so begin this process immediately!

Osteopathic Licensure Application - In addition to the application, the following must be completed before your license can be approved.
  1. Final, Official Medical School TranscriptsYou are responsible for requesting your final official medical school transcripts (must show degree earned or date conferred) or a letter of good standing stating your anticipated graduation date (Note: The letter must not be dated more than 90 days prior to your graduation date) to be sent directly from your medical school to the State of Michigan. We recommend that if your graduation date is set after the first week of May, that you request a letter from your school instead of your transcripts. Otherwise, your transcripts will not be received in time for the State of Michigan to process and approve your license before you begin. Please do not delay in requesting this information. Your medical school must send your final official transcripts or a letter of good standing directly to the State of Michigan Licensing Board, PO Box 30670, Lansing, Michigan, 48909. (If sending by Federal Express, must send to: 611 W. Ottawa, 1st Floor, Lansing, Michigan, 48933).
  2. Certification of Appointment to Training Form – You must complete and sign the first page of the Certification of Appointment to Training Form and return both pages one and two to the Medical Education Department for completion of page two. Medical Education will submit both pages to the State of Michigan. Please do not separate the two pages.
  3. Certification of Internship Form – This form needs to be completed only if you’ve already completed an internship. If so, complete the first page and return both pages to your place of internship. They must complete and mail directly to the State of Michigan.
  4. Verification of Licensure or Registration in Another State – This form needs to be completed only if you’ve ever been licensed in another state. If so, complete the top portion of this form and submit it directly to the appropriate State Licensing Board for completion. Most licensing boards require a fee for processing, so please check with them before submitting.
  5. Fingerprints and Background Checks - All applicants for health professionals license in Michigan are required to submit fingerprints and undergo a criminal background check. Please follow the instructions in your licensing packet. The Michigan Board is not able to accept fingerprints that have been obtained for any other purpose. Your license will not be issued until process is complete.
Allopathic Licensure Application – In addition to the application, the following must be completed before your license can be approved.
  1. Certification of Medical Education Form – You must complete the first page of this form IMMEDIATELY and forward both pages to your medical school for their completion of page two. Your medical school must complete and send this form directly to the State of Michigan Licensing Board at the address located at the top of the form.
  2. Certification of Appointment to Training Form – You must complete the first page of this form and return both pages one and two to the Medical Education Department for their completion of page two. Medical Education will submit both pages to the State of Michigan. Please do not separate the two pages.
  3. Verification of Licensing or Registration – If you have ever been licensed in another state, you must complete the top portion of this form and submit it to the appropriate State Licensing Board for completion. Most licensing boards require a fee for processing, so please check with them before submitting your form.
  4. ECFMG Certificate (if applicable) - Beginning January 1, 2009, all applicants who graduated from a foreign medical school must have verification of their ECFMG certification electronically submitted from the ECFMG office. Information and instructions on how to have an ECFMG status report sent to the Board can be found at https://cvsonline2.ecfmg.org/ImgGenInfo.asp. Faxed or paper copies of ECFMG certificates are no longer acceptable for licensure purposes.
  5. Fingerprints and Background Checks - All applicants for health professions license in Michigan are required to submit fingerprints and undergo a criminal background check. Please follow the instructions in your licensing packet. The Michigan Board is not able to accept fingerprints that have been obtained for any other purpose. Your license will not be issued until process is complete.

Podiatry Licensure Application - In addition to the application, the following must be completed before your license can be approved.
  1. Federal DEA Number – At orientation, you will be provided with a “Genesys” DEA number that is to be used within the parameters of your training program only. If you obtain a permanent medical license and plan to use that license to “moonlight”, you must apply for and obtain your own Federal DEA number.
