Initial Health Screening/Drug Screening

All trainees are required to have an initial health and drug screening provided only by the Department of Employee Health Services at Genesys Regional Medical Center. You cannot begin your training program unless this requirement is met. No exceptions!

To schedule your appointment, please call the Employee Health Services at 810-606-5957, between the hours of 7:00am – 3:00 pm., Monday thru Friday. The appointment dates available are Tuesday, June 12, 2012, Wednesday, June 13, 2012 and Friday, June 15, 2012.

A urine drug test will be performed, so you will need to be able to produce a urine specimen during your appointment. Please plan for this accordingly and consider refraining from using the restroom as least one to two hours before your scheduled appointment.

Please arrive at your scheduled appointment on time. Appointments typically take one to two hours; but please prepare for the appointment taking up to four hours.

Males must be clean shaven for a TB mask fit test.

On the day of your health screening, you will have your picture taken for your i.d. badge, so please dress appropriately.

Employee Health Services is located on the second floor, Suite #2605, between the Cherry and Maple Elevators.

You must bring the following immunization records and titer results with you to your appointment:

Hepatitis B - Series of 3 documented immunization dates
Hepatitis B Antibody Titer - Documented surface antibody titer after completion of Hepatitis B Series
MMR (Mumps/Measles/Rubella) - Documented dates of 2 immunizations or documented titer results for Mumps, Measles and Rubella
Varicella (Chicken Pox) - Either know if history of disease or documentation of two (2) Varicella immunizations, or documented laboratory titer for proof of immunity
Tuberculosis Skin Testing - Documentation of 2 most recent tests
Tetanus (TD, Tdap) - Date of most recent immunization
All prescription bottles if you are taking any prescription medication
