Post Traumatic Stress Disorder Assessment

In some people, an extreme traumatic event, such as a physical or sexual assault, terrible accident, natural disaster, or the sudden and unexpected death of a loved one, may lead to a medical condition called post traumatic stress disorder (PTSD).

Your answers to the questions below can help your doctor or other healthcare professional learn if you have symptoms of PTSD. Have you experienced, witnessed or learned of an extreme event recently? If so, please take the following assessment:

1. Do you avoid being reminded of this experience by staying away from certain places, people or activities?
2 .Have you lost interest in activities that were once important or enjoyable?
3. Do you feel more isolated or distant from other people?
4. Do you find it hard to have love or affection for other people?
5. Do you feel there was no point in planning for the future?
6. Do you have more trouble than usual falling asleep or staying asleep?
7. Are you jumpy, anxious or get easily startled by ordinary noises or movements?
