Bringing Health Care Home
Genesys Home & Hospice Care coordinates patients’ overall care with physicians to provide continuing services in their homes.

Our professional staff cares for patients recovering from acute illness, trauma or surgery; promoting their wellness and independence. We adjust our services to meet the changing needs of the patient. By incorporating our services into the family’s daily routine,we minimize disruption of their lifestyle while promoting patient and family independence. Almost any care provided in the hospital, doctor’s office, clinic or rehabilitation facility is available for homebound patients.

Who Qualifies
Patients must be under a physician’s care and in need of intermittent services. In most cases, the patient must be homebound. We provide services to residents of Genesee, Lapeer and Shiawassee counties, as well as those living in parts of Livingston, Oakland, Tuscola and Saginaw counties.

The Home Care Team

  • Nurses provide personalized care tailored to the patient’s need. Using the latest technology, IV’s, wound and cardiac care are some of ourspecialty teams.
  • Home Health Aides assist with bathing, grooming, hygiene and feeding. Physical, Occupational and Speech Therapists provide exercise, caregiver and patient education, mobility and activities of daily living training to return an individual to independence.
  • Social Workers assist the physician by focusing on the social and emotional factors relating to the patient’s need. They assist patients with financial resources and suggest additional services when needed.
Special Services Meet Special Needs
  • Pediatric Nurses are committed to meet the special needs of children who are sick or injured. They coordinate the home care team to provide the best possible care for medically fragile children.
  • Maternal and Infant Support Services is a team of social workers, nurses and dietitians who provide services to high-risk mothers and infants.
  • Gestational Diabetes Program provides expectant mothers with monitoring and education so mother and baby can have a healthy future. A certified diabetes educator, registered dietitian and skilled nurse will follow the mother through the pregnancy.
  • Tele-health Program is for patients with a diagnosis of cardiac respiratory disease and other illnesses that need close monitoring. The Tele-health machine can monitor and manage patient’s health from their own home 24 hours a day, seven days a week. The machine records vitals and transmits health information back to trained staff in the home care office.
  • Palliative Care Program is for adults or pediatrics who have a terminal diagnosis but still want to receive aggressive treatment. A nurse, social worker and home health aide will all assist with care and medication management.
  • Hospice care allows terminally ill patients and their families to remain together in the comfort and dignity of their own home. The team of physicians, nurses, nursing assistants, dietitians, social workers, clergy and volunteers are dedicated to provide compassionate medical and emotional care. The Hospice Care Center provides 24 hour care in a home-like setting for terminal patients who are not able to remain in the own home.

Serving Patients Where They Live
Genesys Home Care services patients at their place of residence living in the following counties: Genesee, Lapeer, Oakland and Livingston north of M-59, Sagniaw and tuscola south of M-46, and Shiawassee just east of 127.

Most Services Are Covered By Insurance
Both home care and hospice care are less expensive than hospitalization. Most services are billed directly to Medicare, Medicaid, Blue Cross, HMO’s and commercial insurances.
